release of medical information
Can we require employee using intermittent FML to sign a release so we can get relevant information on absences from employee's doctor? Any suggested form?

It's unclear what you mean by "absences from employee's doctor", however, if you're referring simply to doctor's appointments which the employee may have missed, I am unaware of anything in the FMLA which would give you as the employer the right to such information.
