Bill of particulars

Is there a form for bill of particulars?

My case went from general district court to circuit court.

Only in a very generalized sense in that all Bills of Particulars are set in case-captioned style and may be entitled PLAINTIFF'S RESPONSE TO DEFENDANT'S REQUEST FOR BILL OF PARTICULARS or some variations thereof, depending upon which party has requested such a bill(or been requested to provide such a bill).

And under that comes the introductory paragraph explaining to the court who is doing what with this particular BILL OF PARTICULARS, and then setting forth below that in numbered paragraphs the various items requested (or being responded to) in the bill.
