Name Change in Passport

A few years ago I changed my name in my passport at a U.S. Consulate overseas. My passport reflects my new name. No other documentation that I have, drivers license, credit cards, etc., uses the new name. My old name is used on all documentation except the passport. I now want to change the name in my passport back to the old name. What is required to change the name in my passport?

Check with the State Department regarding the rules which govern this situation. (It may be simplest to just apply for a new passport with your old name, if the rules allow for this.)
