Motion to set aside default judgement Because of No service

I recently checked my credit report and had a judgment against me from 3 years ago. I called the court and received a copy of it and it says that the summons was posted on the door where I no longer lived. I was living in Michigan at the time and never received it. I want to file a ''Motion to set aside default judgment'' to at least give me a chance to defend myself since the debt in question is not mine. However I have a form DC-434 to motion the court but it does NOT have an option that says that I wa never served. SO what do I put to let the judge know that the reason I am filing i because I never received the orignal summons because I was not living in Virginia at that time.

As the last option write in "OTHER" followed by the name of the motion you seek to file and the reason for it.
