Thirty years ago a cousin entered my name as a beneficiary on an uncle's burial insurance policy.
This was done without my knowlege or consent. Was that legal?

Probably. Generally the beneficiary is chosen by the person who owns the policy (in this case your uncle). If you are the only beneficiary, and that is not contested, it will be presumed that your cousin added you at your uncle's request. The beneficiary does not need to consent to being named as a beneficiary--you benefit from it, there are no obligations associated with simply being named as a beneficiary.
