Does a commission only paid outside sales
rep still receive commissions paid if on
medical leave?

Yes, assuming that the employee has actually earned these commissions and that there is nothing in the employee's contract with his company governing compensation that would contraindicate or disallow such payment under the circumstance described (in my opinion).
