What form is needed to file a complaint aginst a business man who fails to pay for services rendered?

If you were an employee of this businessman, you can file a complaint with the Labor and Employment

Law Division of the Virginia Department of Labor and Industry. You can obtain the necessary Wage Claim Form from any of the DOLI's regional offices or download the form online. They will investigate your claim and assist you in collecting any unpaid wages which are owed.

If you were not an employee but supplied services to the businessman as an independent contractor,

your best option, most likely, is to sue him in

Small Claims Court for the amount that you can prove that he owes you. You can file your claim for about $25.00, and you don't need a lawyer for

this.(In fact, lawyers are not permitted to represent clients in these kinds of suits in Virginia.)
