Legal Question in Employment Law in Virginia

I worked for a cosmetic surgeon for 3 1/2 years and had some cosmetic stuff done, I signed a form through him that I would stay for 4 years after or I will have to pay back the money for the procedure. Questions is how legal is that form and can he keep my last paycheck

Sounds like a contract, but without actually reviewing the instrument and the relevant facts surrounding its execution, it would be difficult to gauge its enforceablity(in my opinion).
