Delinquent 'adult' in need

I am wondering how to write up an agreement or, what type of form to use for my situation. We have agreed to let a young man come and live with us whom we have known for many years. He has been in trouble with the law for drugs and has had his license taken away for drunk driving. He would like to make a fresh start and move in with us but, I would like to cover my bases just in case he isn't able to handle our rules. I have already told him that his stay is conditional; i.e., random drug testing, has to get a job, will be held accountable for his actions, etc. We live in the State of Virginia and I want to make sure that if he breaks any of the rules, I have grounds to make him leave the house. What type of form would I use and how would I word it?

He is 19 years old and will soon be 20 so, he is an adult, technically.

No form is needed nor even any written agreement for this situation; the young man you've invited in to your home would have no more status than that of a house guest who could be required to leave at any time---and with the assistance of local law enforcement authorities, if necessary. (I might also add, perhaps, unnecessarily, that this proposed invitation would appear to have vast potential for lots of trouble---for you and members of your household.)
