past due rent

how do i write out a five or three day notice on tenants who are pass due in rent, damaging unit and have other people not on lease living in unit?

NOTICE TO TENANTS OF UNIT #_______

You have five/three days from this notice date__________to pay or quit from the above referenced unit/apartment.

All rent arrearages must be paid up as well as any current rent now due by the above date.

Furthermore, the other material breaches of your lease which are apparently now occurring must cease immediately, which include the continued unauthorized presence of the persons who are now living in the unit and who are not on your lease and the damages of various sorts which are currently being done to your rental unit and for which you will be held responsible.

If you are not in complete compliance with all of the conditions listed above by the referenced date, action will be taken in court to have all of you formally evicted and a judgment entered against you for all monies owed.
