Products liability cases can be quite complex, and I would suggest you not try to do it pro se. Lawyers generally do these on contingent fees, so you don't pay legal fees (and often expert witness fees, which can be very high) if you don't recover. As to your specific question, there is no "form" letter to notify the seller/manufacturer of the laDDer, but you do want to send notice to preserve your warranty claims. Note that VA does not have strict liability for defective products like most states do - you have to show negligence or breach of warranty, and contributory negligence is a defense on negligence claims.
