Is there a form that I can send to my last tenant that explains the money owed for damages that were incurred while living at the property?

There is no official form that is agreed upon or required. It does not have to follow any specific format I am sure there are many examples out there that different landlords are using. But there is no "one" form, and no particular form or format is required.

Just be detailed and complete. And pay very close attention to what the lease says. Make sure you understand what damages are their responsibility under the lease and standard practice. Then when you write the list, make sure you are clear enough that it is (hopefully) obvious that it is an item which they will be responsible for.

Notice also that you have to pay attention to the responsibilities to allow them to have an inspection before they move in and after they move out. THey are supposed to be able to make notes of what was ALREADY damaged when they moved in. If they did this, look it over carefully.
