How can an order or document be formally requested from a clerk of courts?

Just go to the clerk's office of the court which

issued the order or where the document was filed

with the name of the case(and case # if available) and request a copy of the order or document.

The requester will usually be required to pay a small fee for this service.

Well it would help to be more specific and detailed in your question.

You have to request an order from the judge. You would have to file a motion and set it for a particular date. You have to ask the Clerk by phone for an appropriate date when motions can be heard. And then you have to ask the judge LIVE, in person, for an order to be entered.

As to a document, I don't know what you are looking for. You can go and ask to look at the file for any case, and just look through the file.

If you are asking for a blank form you typically need to go there in person and get a blank form. That's a pain. Many of them, however, can be found at www.Courts.state.va.us

However, if you want some other document like an abstract of judgment (a certified copy of the results of a closed case) or something, you can do that simply by writing the clerk a letter. They will probably call and tell you that there is a fee of some kind. You can call first and discuss it.
