Request for trial in abstentia
Is there a prefered format to address the court to be tried in abstentia for a traffic violation?

No, just ask the court for their form
and fill it out and submit it.(Unfortunately, in most cases, making such a request to be tried in absentia(your absence)is tantamount to asking to be convicted of the infraction or violation.)
