Advance Medical Directive

I have a draft of an Advance Medical Directive. However, it is not as strong as I would prefer. I am told that if I make it stronger, it may become void. It was suggested I have supplemental document expressing my strong wishes.

I want no life prolonging procedures performed unless I am likely to fully recover within a month.

How can I effect this?

You are told by whom? Without reviewing these papers, I would think that it would be difficult to provide any reliable advice on this matter.
