Incident Report for Waste Management Inc.
On November 12, 2008 I--was coming from the kitchen with a cup of water. I entered back into my room where Tracy Harden and Nichole Anderson were sitting. I plugged my headset back into my cord that�s attached to the amplifier; hit �Auto-in� to receive calls. A call came thru and (2) static shocks came thru the line going into my ear. It scared me so I jerked, snatched my headset off, grabbed my ear and went straight to Felicia Garrett, my Supervisor. I was in a lot of pain; there was a burning sensation in my ear and sharp pain shooting. I kept getting hot inside my ear and thru-out my body.
There is also new construction being done at this location along w/electrical installation. Also found out today 11.17.2008 that the amplifier that was attached to my headset the batteries on the inside was corroded.

If you have a verifiable injury received in the course of your work, you should file a workman's compensation claim.
