Legal Question in Business Law in Washington

I am a physical therapist and one of my patient's attorney has asked me to send a letter/form to them stating that I will be paid when the patient settles with the insurance company. Is there a form I can use for this? I looked in the Find Forms for ''lien'' and there was no match. Thanks for your help.

You need to file a medical lien to insure that your rights are protected. There is a certain procedure to this and you should seek out an attorney to make sure that the correct form is used and that all of the filing requirements are met.
