Is there a way to protect myself from his ongoing medical bill's? Or once we are married am I responsible for all his bill's he accumulates once we are married?

Re: Fiances medical bill's
If you are married, you can file a legal separation and establish a date of separation.
If you are not married you are not responsible for his bills.
You ARE responsible under a joint and several theory - and community property - for bills he incurs after you are married.
I hope this helps. Your question is not clear, the more I look at it!
