I need to reply to this summons in my defense, is there a form to use?

Not really. You can go to the courthouse law library or to a legal messenger company and get something that is usable.

If you need help but can't afford it, go to the County Bar Association and ask if they have a volunteer legal services group, or call CLEAR to get to Northwest Justice.
