Name change of minor child

I would like to find out about the process of changing my Daughter's name. She has her Father's last name, but he has no seen her since she was one years old. Do I need his permission to chage her name? I have no way of contacting him and he would most like ly not give permission. If I do need his permission, is there a way around this because of his lack of contact?

This can be done, but won't be just a matter of showing up at the courthouse and having the judge enter an order.
