custody of a minor

My husband and I have been asked to take full custody of a minor that is not blood related, the mother has agreed to sign her over to us. How should I word it and should I have it notarized? The mother does not want the father around the child (he is currently in prision), do I need to have that added into anything I have her sign?

If this is more than just a short term arrangement you should consider doing a guardianship for the child.
