legal last name change

My daughter just turned one year old. Her father signed her birth certificate but hasn't been in her life since she was born. I gave her his last name but since I had to get a restraining order against him, and i have full custody. How can i change her last name without someone else adopting her.

For starters nobody else can adopt her without termination of the father's parental relationship with the child. It is difficult to change a minor child's name without the consent of both parents.
