Legal Name Problems Due To Birth Certificate/Adoption

I was adopted when I was around 3 yrs old...but before my adoption was finished...my adopted father died suddenly. My adoptive mother also lost her brother at the same time. She was so traumatized that she never finished finalizing everything. My birth mother has passed away also. My birth cetificate shows my birth name...social security is adopted name and I have been married twice...how do I change the name on my birth certificate to my adopted name? How do I get a passport when I can't prove who I am in regards to my birth certificate and adoption? Can you help steer me in the right direction?

Go to the District Court in the County where you live and ask to have your name changed formally to the name you are known by now. Then you can ask to have the Department of Health issue you a new birth certificate.
