how do gain medical power of attorney of my 13 yr old daughter bi-polar child

You all ready have it, as you are her parent. There is nothing you need to do to memorialize this, unless you are not the primary residential parent, in which case there is something more going on here.
Persons younger than age 18 are, in the eyes of the law, "infants". They can make no decisions for themselves, and are dependent on their parents to act. They cannot legally act on their own behalf, you have to do things for them.
Also - barring a diagnosis from a well regarded child psychiatrist, please do not leap to the conclusion that your child is bi-polar. Tennagers are naturally wierd.
