Mother directs child to use stepfathers Last Name
My daughters mother is directing her to identify herself with her stepfathers last name.
I am an extremely involved parent, Is there any legal way to prevent this?
Medical insurance is carried under my last name.. One concern is confusion in an emergency that may delay treatment.

Your question is a little bit ambiguous. How is she "directing" you daughter to identify with the stepfather's last name?
Do you have joint decision-making authority? If you do, the step-father is not normally allowed to make any decisions affecting the non-emergency health care, education, or religious decisions affecting your child unless you have participated in the decision-making with your ex and approved this level of the father's involvement.
If school records are being entered with a name other than the child's real name, you may have grounds to complain that this is not appropriate.
If medical records are being entered under the wrong name, you may ask the court to direct the ex-wife to cease and desist.
