Legal Question in Family Law in Washington

non parent custody

Someone has asked me and my husband to take temporary custody of her son. The child is 6 and she does not know who the father is. He has lived with us on and off from the time of his birth. She is moving out of the State for a while to try and get reestablished so it could be up to a year or more. What kind of forms would we need to fill out?
