Passport for child

I want to obtain a passport for my 12 year old son. His father and I were never married. His father signed a paturnity affidavid a month after his birth and appears on my son's birth certificate. I have not spoken to his father since the birth. he has choosen not to be in my son's life. I can not obtain consent from his father for the passport. What should I do?

File a parenting and support action and list as part of the relief requested an order authorizing you to apply for a passport for the child without father's consent.
