In order to demonstrate adequate cause to modify an existing parenting plan, you have to be able to show a substantial change in the circumstances of the non-moving party or the child that would warrant a modification.
If the doctor is willing to write, that would be helpful but not dispositive.
Don't ask the child to write a statement; judges hate that.
Modifications require a showing of substantial evidence in order to proceed, because the court is loathe to toss out a parenting plan and make a new one and move children. Your case will also be a substantial hardship on the child's mother, so be prepared to pay her attorney fees and costs in the event you are unsuccessful.
Hope this helps. Powell

If the mother is not agreeable, you go to court and file a Petition for Parenting Plan or a Petition to Modify an existing parenting plan.
Normally an attorney's help is best, but the forms are available on the WA Supreme Court website.
