Legal Question in Medical Leave in Washington

Do I need to re-apply for FMLA if the time off needed is longer than the original ''anticipated'' time? I had surgery on my shoulder and when I went for my follow-up exam, the Dr. said I needed to take at least another 4 weeks off. I have faxed the documentation from my Dr. extending my leave. HR now says I need to get additional approval for this extention.
