Insurance Case
While eating at a restaurant I bit into some hard object and broke my tooth. The restaurant insurance asked me to submit the bill plus my dental records. Am I responsable to facilitate information other than the bill or is their responsability to gather additional information?

There are two ways of going about this: (1) you can sign a release authorizing the insurance company to obtain your records and (2) you can get the records yourself and provide them to the insurance company. In either case, you might want to consider legal representation.
