Hurt at home
I was hurt at home 5 weeks ago my employer is wanting me to sign a release form and I dont understand it. Please help!

Definitely, don't sign anything you don't understand. If you can't make sense of what the form says, have an attorney review it for you. (Don't take the employer's word for what it says.) This should not be an expensive proposition, especially if the form is short.
That said, until you get the form translated into "regular English", don't assume anything one way or the other about it. The employer may be trying to take advantage of you, but the employer may also simply be concerned that you might try to claim injury on the job to get L & I benefits. If you were truly injured at home, you don't qualify for those, and if you file for them, this will increase the employer's costs unfairly. Having you sign a release form is probably not the right way to handle that, but the employer's intentions would be innocent in that case.
