Typically we are dealing with "assignments" of personal property and quitclaim deeds with real property. With accounts you must change the name on the account. It is critical that you do all this and correctly or the trust is worthless. I probated three estates last year that had trusts because the trusts were improperly funded. Don't make that mistake -- consult with an attorney.
