My mother deded me her home, & if anything should happen to me as far as death or incapasatated, I want the home to be given back as a last will. I have One child that should have no interest in the home.Is there a form we can use for this?

No. There is no form that solves this for you.
Contact your County Bar Association. They will have a lawyer who will draft a will for you. If there is an income issue, usually there is a program where the lawyer's fee for drafting is $10.00 or so (at least that's true in Pierce County).
