Protecting our Granddaughter

If my son and his x-g-friend willing sign over full custody of our granddaughter to us, my son is a drug addict and the mom just does not show any interest at all in being a mom. How do we protect ourselves and our graddaughter legally through this process? What needs to be done to make this a smooth process? They live in wv and we live in va

You as the child's grandparents should file for custody in West Virginia which would appear to have jurisdiction over this matter.

Neither your son nor the child's mother are legally empowered on their own merely to sign over custody of this child to you. This must be done through a court with proper jurisdiction(power)to enter a formal order awarding you full custody of the child and, ideally, addressing any other attendant matters such as child support and visitation of the parents at the same time.
