Reimbursement for incurred cost of echocardiogram

I had an echocardiogram completed for the Phen-Fen settlement and my insurance paid for all but $300.00 which came out of my pocket. In order to get reimbursed from AHP, the cardiologist who read the echo is required to complete a form that I then send in. The cardiologist's office refuses to complete this form stating that he did over 1400 and to complete this many is impossible. Meanwhile, I am out 300 dollars! Is he legally responsible to complete a form about this or do I need to take him to small claims court? If so, do I have justification?

Perhaps they will send you the form and you can complete it yourself with the help of his office staff. He could then sign it and you can mail it in.

Otherwise we recommend you go to small claims court and file a complaint stating your concerns.
