Name change

My 15 year old daughter wants to change her last name to my new married last name. Her father has been verbally abusive to her and she no longer wants to be associated with this behavior or family. Can she do this without his consent in the state of Wisconsin?

Re: Name change

A name change for a minor requiress the consent of both parents or an order from the court. If you cannot acquire consent of the other parent, you would have to file a Motion and the determination on whether the name change occurs is based on what the court believes is in the child's best interests.
