my ex has until a certain day to pay me for unpaid medical and dental bills or a judgement will be placed on him. Is that something i have to do or the courts do? What is a judgement

Judgment for moneys come at the end of a litigation over those funds.
If the court has already ordered him to pay you can file for a judgment with the court, OR just file a Notice of Motion and Motion for Contempt and ask them to put him in jail for non payment. That usually has more power over his response and payment.
If all you get is a Judgment then you have to enforce it, which is far more painful than it is sometimes worth.
