Selling house in WI - what form to use for buyer to make offer?

I am selling my house and I have a potential buyer. I am wondering what form(s) to use to accept the offer.

If the offer is not on a standard Offer to Purchase form, you should have the buyer resubmit it on such form. That form covers most of the possible situations that could occur. It also provides for acceptance right on the form.
