Question about guardianship

I would like to know if there is a specific form I can fill out in regards to granting custody/guardianship of a child in the event of both the parents death. Is this something that needs to be done through the court system or can the document be presented when the time comes? Is something liek this covered in a standard will? Thank you.

In Wisconsin, a guardian for a minor is nominated in a will. The final decision, after both parents die, is up to a court. In such situations, the child becomes a ward of the state, and the language in the will is considered, with the best interests of the child becoming of paramount importance. See a qualified attorney to be sure the will is drafted appropriately.
