My Grandmother is 86 years old and I'm her oldest granddaughter and both of her kids are not with us anymore. I need to know how can I get power of attnory or what would be better for the both of us, she needs medical help and I need help getting her things in order for her.

If she is competent to sign a power of attorney, if she desires such documents and wants you to assist her in that capacity then speak to her and contact an elder law or estate planning attorney. If she is not competent to make decisions (such as needed along the lines of making a contractual agreement) see an attorney about a conservatorship.
