Legal Question in Family Law in California

1) What is the difference between "In the Matter of ____", "In re _____" and "Ex parte _____" and other versions of case names?

2) Do judges choose the those names or are there common law rules for them?

3) How are family law case names decided to be closed or not? Example, "S v. S" or "Stevenson v. Stevenson"

4) Why is it permissible on appeal of a sealed or closed family case to unseal or open it? Same example, S v. S in Kentucky became Stevenson v. Stevenson on appeal. Seems like the privacy interest would stay the same.
