Is a notary needed for filing?

I am filing a Request for Special Notice, this will be sent by mail to the clerk for filing as well as a copy to all parties related to this case.

If the form does not state the need for a notary, is it something that should be done? My form is DE-154 its 2 pages and can be seen at this url:

The form does not require a notary.

As a general rule, if a form requires a notarized signature, there will be a notary's jurat or some other indication that this is where a notary is to sign.
