Trustee named in life insurance benefit

In our separation agreement my ex-husband promised to provide a legal document stating that he has life insurance on himself with our two kids as beneficiaries and me as trustee. To date I have been unable to obtain such a document from him. He gave me a document last week, but it does not list trustee nor the amount of the life insurance. He said it is a "given" that I am trustee since the kids live with me. Is that the case? How can I get him to provide me this document since he claims the benefits office has no such documents and the benefits office refuses to talk to me. Thank you for any help in this matter.

Since he agreed to do it, that was a part of the order, consider filing a contempt of court motion, which is how court orders are enforced. An attorney will be able to help you with this problem.
