The Fact Information Sheet serves several purposes, one of which is to help determine whether there may be a basis to file a garnishment against your wages or your bank account.

If you elect to not include your wife's information, you run the very real risk of the creditor's attorney asking the Court to hold you in contempt. If the creditors attorney brings a motion to compel you to respond to any part of the Fact Information Sheet, he may be awarded attorney's fees. The best course of action is to complete the form honestly, truthfully, and legibly and return it to the creditor's attorney within the time prescribed.
