Cancellation of a contract

I signed a contract with a local health club. I have not used the club for almost a year and would like to cancel my contract. According to the contract I either must have, a medical release stating that I am disabled from using the facility for 90 consecutive days or, I have moved 30 miles from any of franchise health club facilities. I am not moving and I am unable to obtain a medical release, because I am healthy. The contract also stated that if I were dead, that would also release me from the contract. Can I use the Cancellation of Contract No Fault form to end my contract ???? Or, do I use the Cancellation of Contract At Fault Form ???

No way to advise withoug review of the documents. How much is at stake? Can it be assigned or sold to another person?
