Legal Question in Wills and Trusts in Indiana

My father is 78 and has recently been diagnosed with dementia. He owns a house and has a savings account, which were always meant to be his children's inheritance. We are all grown. Can he sign his house over to us, and write us each a check, which he is willing to do, if he needs to go to a nursing home? He worked all his life and was in the Navy for 20 years, so the money he has came from his pension and social security.
