Getting custody of my kids back from their dad

My friend is getting her children back from their dad. She needs a legal form for him to sign. Where can she find this?...He money is tight and cannot have a lawyer draw it up.

There is no such "Form". These documents are drawn up on a case by case basis. No two are alike in that each situation is somewhat different. This "document" and the change of custody MUST be approved by the court awarding the original custody order.
