Disclosure notice; refinance

The terms of my mtg changed when citifinancial refinanced the same mtg loan. I signed nor initialed any disclosure papers and only found out about the changes when i called the mtg company. The terms -not rate- are thins i never would have agreed too. This has been going on for about 12 months. Whom should i contact?
