Name change of a minor

I put the father's last name on my daughter's birth certificate, he has been gone since she was 3 days old, I have not received any child support for more than 3 years. I want to change her name, on her birth certificate, is this possible or can I only change her name and amend her birth certificate?

You would need to change her name in the probate court in your county.
