Health Insurance for Employee out on STD

If an employee is out of work with a short-term disability, is the employer required to continue health insurance coverage? Also, is the employer able to require that the employee continue to pay the employee's normal co-pay that is required according to the plan? Or is the employer only required to offer altnerate Cobra insurance coverage, which is to be paid 100% by the employee?

Short term disability is a matter of contract between the employer and employee, and there is no federal or state law requirement that health insurance be continued at the employer's expense.

Employers are required to offer to continue medical benefits to employees at the employee's expense, but the employers are not required to pay any amount toward this coverage.
